Registration: Required Documentation

  1. Proof of residence: utility bill or contract on home/apartment
  2. Birth certificate (for help obtaining records, call 458-7111)
  3. Social security card (for help obtaining records, call 800-772-1213)
  4. Immunization records (requirements listed below; for help obtaining records, call 458-7111)
  5. Last report card
  6. State of Texas Academic Achievement Record (AAR) / Transcript
Immunization Requirements:
  1. All children under the age of seven must have a minimum of four doses of DPT/DTAP, with one dose being on or after the fourth birthday.  Children age seven and older must have a minimum of three doses of DPT/DTAP, with one dose being on or after the fourth birthday.  A booster is required in ten years.
  2. All children must have a minimum of three doses of Oral Polio Vaccine; with one dose being on or after the fourth birthday.  OPV is not required after age 18.
  3. All children must have one dose of Measles, Mumps, and Rubella Vaccine on or after the first birthday.  A second dose of Measles is due by the child's twelth birthday.  All students born on or after 9-2-91 and entering school for the first time, must have their second dose of Measles Vaccine before entering school.  All pre-kindergarten students must have their second dose of Measles Vaccine as they reach their fifth birthday during the school year.
  4. All students born on or after 9-1-92 and entering school for the first time must have completed the series of three Hepatitis B vaccines.
  5. All students under the age of five must have had one dose of Hib Vaccine after the age of 15 months or a series and booster completed by 15 months.
